Abstract
The midbrain and mesodiencephalic junction contain the key structures involved in the premotor control of vertical and torsional eye movements. These areas also have the neural structures for vergence and descending fibers controlling horizontal gaze. Lesions involving the midbrain or mesodiencephalic junction disclosed horizontal gaze palsy as well as characteristic vertical gaze palsy. We describe a case of bilateral vertical gaze palsy with horizontal conjugate gaze palsy due to unilateral midbrain infarction.
